§ 8-6. Application for license.

Each person who is required to obtain a license under the provisions of this part shall make application in writing to the building official on forms provided by him. The application shall be signed by the applicant under oath and shall contain the following information:

(1)

The full name, sex and age of the applicant.

(2)

Residential address at the time of application.

(3)

The name under which the business is to be conducted.

(4)

Location of place of business for which the license is sought.

(5)

Type of license being sought.

(6)

Residential or cellular telephone number.
